Step 1 – Enroll a NEW AmeriPlan Deluxe Plus Member Only, between Monday, August 27th and Labor Day, Monday,
September 3rd on a standard Deluxe Plus application online and receive your regular as earned
commission of $15.98 or on a TAM application (if you have one) and get the $75.00 TAM Bonus. You will
then be able to download a Special SOGO Application PDF from your BackOffice. This SOGO Application
is good for one Deluxe Plus Member (Cannot be used for enrolling a Benefit Consultant).

Step 2 – Enroll a NEW AmeriPlan Deluxe Plus Membership on the Special SOGO Application by Sunday,
September 30th at 5:00PM C.S.T. and you will receive a Special SOGO Bonus of $50.00.
